Treatment contract
I confirm that I am the owner of the animal or acting on behalf of the owner and that I will bear the costs of the treatment.I authorize the practice, if necessary, to engage third-party services (e.g., laboratories) on my behalf and at my expense.I will pay for the treatment immediately after the procedure in cash or by debit/credit card.I understand that the practice may retain my animal until full payment is received (right of retention pursuant to Section 273 of the German Civil Code).
Declaration of consent for data usage
By signing this form, I consent to the veterinary practice Vet. Med. Praxis Bitburg (responsible: Zlatko Jakovoski) collecting and processing my personal data provided on the registration form for the purpose of carrying out a veterinary treatment contract.
